Welcome to our center for application forms and instructions. We will be adding to this page in the future, but have the following forms available for you to download now. After printing out the form and completing it, you can fax it to us, mail it in or hand deliver it.

| Crime Statistic Request Form | Citizens can submit the request form for crime statistics for the City of Brea or Yorba Linda. | |
| Film Permit | Live performances need to be presented in an appropriate venue with the required permit. Film crews are required to have a permit before filming anywhere in the City of Brea. |
