Case Management Services
Case Management is dedicated to the achievement of successful aging through advocacy, assessment, resource facilitation and management of service/program options to meet your health and wellness needs. Home visits and telephone calls are provided in order to assess and reassess needs to determine the appropriate community resources for you or your loved ones, such as transportation programs, home delivered meals, friendly visitor programs, and utility discount programs to name a few! Courtesy home safety assessments may be conducted, as well as recommendations of assistive devices to help individuals live safely and independently at home. Based on one’s income and available program funding, assistive devices/medical durable equipment may be purchased to help the client sustain the abilities to accomplish basic and instrumental activities of daily living. 

Eligibility to be a Case Management client: Brea resident, 60+ years of age, and client consent.  

Equipment Lending
The Mobility Equipment Lending program enables Brea residents to borrow assistive devices for older adults in the community — free of charge. Devices include, but may not be limited to: walkers, canes, wheelchairs, transporters, raised toilet seats, commodes, grab bars, bedside assist rails, and more. Also available for lending are CCTV magnifiers for the vision impaired. These items are donated, therefore most have been used. Eligibility Criteria: 60+ years of age and Brea residency. Signature on the Brea Senior Center Loan Form is required.

Personal Care Supplies

The cost of adult incontinence supplies can be challenging for those on a fixed income. Intended for low-income, older adult Brea residents, if you are having difficulty accessing supplies, call and let us know of the product type and size you need. Availability of supplies depends on our inventory, which is gathered from donations throughout the year. For more information, call 714-990-7781.

Gift Shop 

Open Monday through Friday
9:00 - 11:30 a.m.
The gift shop has a large assortment of quality items available at bargain prices, including: jewelry, stuffed animals, wicker baskets, assorted handcrafted items, and lovely gift items. Donated items gratefully accepted during gift shop hours.

Housing Assistance

The City of Brea may be able to provide limited assistance in the area of housing. Please review for qualifications and criteria.

Legal Aid

4th Thursday of Each Month

Medicare & Health Insurance Assistance

2nd & 4th Wednesday of Each Month
9:30 - 11:30 a.m.
Appointment Required
This service benefits anyone who has questions or needs assistance regarding Medicare, HMO’s and secondary or long-term care insurance. Provided by Health Insurance Counseling and Advocacy Program.

Shopping Trips

Mondays - 12:30 p.m.
The Brea Shuttle provides free transportation for Brea residents to Ralphs, Rite-Aid, Target, Wal-Mart, Sprouts, Albertsons, Trader Joe’s, and the 99 Cent Store, local banks, and the Brea Mall on Mondays. Restrictions apply, call 714-990-7754 to register.
